Targeted Case Management
Our Case Managers offer support to children age three through twenty who have been diagnosed with mental retardation or autism. The Children's Case Managers coordinate the supports for the child, to best meet the child’s emerging social, educational and therapeutic needs through creative, strength-based service planning.
At Independence Association, our Adult Case Managers support people over the age of 18 who similarly have a diagnosis of mental retardation or autism.

- The provision of on-going monitoring and reviewing the individual’s service plans to ensure the delivery of quality targeted services.

Advocacy, networking, and resource development support ensures we meet the identified needs of both the child and family.
- Collaboration between our Case Managers and other involved service providers to ensure continuous
supportive contact.
- Transition and discharge plans are developed with individuals to ensure follow-through.
Our Case Management Program’s Mission is to help children and adults actualize long-term goals with aspirations by empowering individuals with the knowledge and resources needed to self-advocate and use all available community supports.
Referral Process
Referrals to our service can be made by parents, providers, or other professionals who are working directly with the child.
Eligibility
Supports are available to children and youth, 0-20 years old with a developmental disability, cognitive impairment or a mental health diagnosis.
Related Supports
In addition to case management there are numerous other support services available to children with special needs and their families. These supports include, but are not limited to, respite, family and individual therapy, in-home supports, medical services, parent support groups, and employment and educational supports.
